Family Yahrzeit Information Form:

If you would like the office to inform you of upcoming yahrzeits of your loved ones, please download and mail in the Yarzheit Information Form.

A Note About Avodah (Service) Hours:

In order to keep track of your Avodah (Volunteer) Service Hours during the year (we ask that you contribute 10 hours), we have provided you an Avodah Service Hours Form.  Please print it out and keep it in your files to track your volunteer hours during the year.  At the end of the year, or when you have completed your 10 service hours, please return it to the Kehilla office.
